Non-Resident Alien Employment Approval Form

Hiring Department, please assist prospective employee in filling out this Non-Resident Alien Employment Approval Form.

This form should only be given to students and applicants who are not Permanent Residents or Citizens of the United States. International students are limited to 20 hours of on campus work per week while school is in session. This form will allow us to keep track of their on-campus employment.

After filling out Name and Social Security Number of the potential employee, enter the Hiring Unit and Fax Number of your department. Then complete the sections below with the student.

  1. Check the appropriate box if you are hiring a Student Employee, or Non-Student Employee.
    • Student Employee: a full-time student at UALR working part time as extra labor.
    • Non-Student: a new faculty, staff or research scholar, holding an Employee Authorization Document (EAD), H1-B visa, or possibly a J-1 Research Scholar visa.

    For questions regarding the status, please contact the International Student Services office 569-3582. If you checked Student Employee please complete the section below, Current On-Campus Employment.

  2. Current On-Campus Employment. Check all that apply. Ask if the student is employed anywhere else on campus. If the student is not working, check “Not Employed.” Call or fax Aramark, UALR Bookstore or named UALR department to verify the number of hours per week the student is working. Add the total number of hours worked on-campus.
  3. Have the student check the tax status box. If there are any questions regarding tax status leave the space blank and International Student Services will assist the student in making that decision.
  4. Have the student sign and date on the signature line.
  5. Fax the completed form to International Student Services. You will receive this form back by fax, signed on the Designated School Official (DSO) line by Jim Felton (phone: 569-3582, fax: 683-7567) and number of hours the student is approved for employment. At this time you can forward the PAF to Human Resource Services. Keep this form in the employee’s record.

Each time an international student is employed or an international student’s employment is altered, i.e., the number of hours of work per week is raised or lowered, the employer must complete a new Employment Approval Form for International Students.
