- Registering for Services
- If you have never registered with the DRC before and would like to sign up for services, use this form to start that process. If you have ever met with our staff before and filled out any paperwork, don't register again! Call our office if you have any questions - 501.569.3143 (voice/tty).
- Request for Books in Alternate Format
- If printed text is a barrier and alternate formats (e.g., braille, electronic text, etc.) are preferred, use this form to request assistance.
- Request for Faculty Notification Letters
- This form is to be completed if you would like letters for your instructors from our office. Please fill out this form as soon as you register for classes.
