End of Program Verfication Form  - For current UALR graduate or undergraduate students who wish to begin a new program of study or are finishing their degree.

Curricular Practical Training (CPT) Application - For students in F-1 visa status who have been enrolled full-time at UALR for one full academic year. CPT authorizes students to take part in off-campus internships relating to their degree program. The internship must either be required by the student’s degree program or the student must receive course credit for the internship.

Optional Practical Training (OPT) Application  - For students in F-1 visa status who are graduating from UALR. OPT authorizes temporary employment that provides an opportunity to apply knowledge gained in the classroom to a practical work experience off campus.

Concurrent Enrollment Form - For students who wish to enroll at two USCIS approved institutions at the same time. UALR students who wish to enroll at another school for a portion of their full-time enrollment must receive the written permission of the International Student Advisor before enrolling in courses at the other school.

Program Extension Application  - For F-1 or J-1 students who wish to extend the authorized period of stay within the U.S. in order to complete original study objectives before the expiration dates on the current I-20 or DS-2019.

Transfar Out Request Form  - For students in F-1 visa status transferring from UALR to another university or institution in the U.S.

Transfer In Request Form  - For students in F-1 visa status transferring to UALR from another university or institution in the U.S.

Change of Address Form  - Immigration law and regulations require that all foreign nationals, including lawful permanent residents, students, visiting scholars and visitors report each personal change of address with the US Citizenship and Immigration Services (USCIS, formerly the INS) within ten (10) days of moving. To report your change of address, please fill out this form and submit it to the Office of International Services for processing.

Change of Educational Level  - For students who are completing one level of education (i.e. bachelors, masters, etc.) and will be starting a new program at a different educational level. This is used to verify that the student has been accepted into the program so that a new I-20 can be created.

Reduced Course Load  – For students who have had to take a reduced course load and have spoken with the International Student Coordinator and their Advisor. This form is used in conjunction with the Reduced Course Load function in the SEVIS system that is used to report international student activity to Immigration and Customs Enforcement.

Request for Invitation Letter  - For students who are graduating and wish to invite parents and/or relatives to the United States to attend the graduation ceremony.
