End of Program Verification - Expected graduation date
You must provide an expected graduation date (please give date) on the Verification Request Form and turned it into the Office of Records and Registration for processing.
Verification of Enrollment
Please specify semester and year on the Verification Request Form and turned it into the Office of Records and Registration for processing.
Letter of Good Standing
Please complete the Verification Request Form and turned it into the Office of Records and Registration for processing. This is also the form for international students who wish to enroll at another institutions at the same time. UALR students who wish to enroll at another school for a portion of their full-time enrollment during Spring and Fall semester must receive the written permission from the Office of International Services before enrolling in courses at the other school. *No verification will be issued for a student whose financial obligation to the University has not been satisfied.
Change of Address Form
Immigration law and regulations require that all foreign nationals, including lawful permanent residents, students, visiting scholars and visitors report each personal change of address with the U.S. Citizenship and Immigration Services (USCIS, formerly the INS) within ten (10) days of moving. To report your change of address, please login to your BOSS account to “Update Address and Phone Number.”
Name Change
International students who need to change their names in UALR’s records should consult with the Registrar, who will specify appropriate documentation.
Transcript Request
A transcript can only be released with the written authorization of the student. Identification will be required for transcripts picked up in person. Please complete the Transcripts Request Form and turn it into the Office of Records and Registration for processing.

Curricular Practical Training (CPT) Application
For students in F-1 visa status who have been enrolled full-time at UALR for one full academic year and are looking for information concerning CPT. CPT authorizes students to take part in off-campus internships relating to their degree program. The internship must either be required by the student’s degree program or the student must receive course credit for the internship.
Optional Practical Training (OPT) Application
For students in F-1 visa status who are graduating from UALR and are looking for information concerning OPT. OPT authorizes temporary employment that provides an opportunity to apply knowledge gained in the classroom to a practical work experience off campus.
Program Extension Application
For F-1 or J-1 students who wish to extend the authorized period of stay within the U.S. in order to complete original study objectives before the expiration dates on the current I-20 or DS-2019:
Financial Guarantee
All international applicants on F-1 or J-1 visas must provide a Financial Guarantee to show evidence of sufficient funds available for their studies before the I-20 and DS-2019s can be issued.

For students in F-1 visa status transferring from UALR to another University or institution in the U.S.

F-1: Dependent I-20
Request to bring in spouse or an unmarried minor child (<21 years of age) of an F-1 student.
Change of Educational Level
For students who are completing one level of education (i.e. bachelors, masters, etc.) and will be starting a new program at a different educational level. This is used to verify that the student has been accepted into the program so that a new I-20 can be created.
Reduced Course Load
For students who have had to take a reduced course load and have spoken with the International Student Coordinator and their Advisor. This form is used in conjunction with the Reduced Course Load function in the SEVIS system that is used to report international student activity to Immigration and Customs Enforcement.
Request for student letter
To request a Visa renewal letter (or Travel letter) to the U.S. Embassies abroad, to the Social Security office, or other letters.
New International Student Orientation
OIS plans the required mandatory orientation program each semester for incoming international students. Important information about academic and immigration policies, as well as practical matters such as housing, transportation, banking, and health insurance are covered during the orientation. Orientation is critical, and all new international students must attend one of these two sessions. It is also a wonderful opportunity to meet and make friends with other new students perhaps from your country and from all over the world!
Departure Form
This form is used to notify OIS that you intend to leave and no longer needs OIS to maintain your student immigration status through the University of Arkansas at Little Rock. Without notifying OIS that you are leaving the U.S. or will not continue your visa sponsorship, you may have problems maintaining a new status in the U.S., lose possible immigration benefits, or create confusion over your student status with UALR. Please come in and speak to us to discuss your personal circumstances and available alternatives.
Invitation Letter Request
For students who are graduating and wish to invite parents and/or relatives to the United States to attend the graduation ceremony. For use by students who are covered by UALR immigration only.
UALR Student Immunization Form
This form is for students to complete immunization requirements in their home country or previous universities before attending UALR.
Pharmacy Reimbursements
Students can now use the Pharmacy Reimbursement Claim Form to send their pharmacy receipts to Lewer for reimbursement instead of coming to the office if they so choose. Simply fill out the form, follow the instructions listed on the form, and send the packet to the address provided.
Immigration Check-In Reporting form - This form is used for the required international student check-in procedure every Spring and Fall.
