19 County Chronic Disease Survey

A survey of approximately 12,400 Arkansas residents in each of the selected Arkansas counties. This survey is being conducted in order to understand a variety of general health practices, including information on health risk behaviors, preventive health practices, and health care access primarily related to cardiovascular disease and diabetes.

Arkansas Tobacco Quitline Evaluation Survey

This is an ongoing survey conducted as a four, seven, and thirteen month follow-up evaluation for those individuals who had enrolled in the Arkansas Tobacco Quitline program.

Community Engaged Study of Social Networks and Long-term Care

This was a survey that was done in order to examine the role of social networks with regards to access to and use of long-term care for persons with long-term care needs. There are three phases to the survey: 1) A self-administered iPad survey of Community Connectors, 2) A telephone survey of individuals identified by the Connectors as persons with long-term care needs, and 3) A telephone survey of individuals in the community with long-term care needs.

ASDA Survey of Arkansas Dentists

This was a web and telephone follow-up survey of approximately 987 currently licensed dentists in Arkansas. The survey was for the purposes of gathering assessment information to be used by the Arkansas State Dental Association in order to gain insight into the current dental workforce as well as determine facility structure and services across the state.

ACHI Tobacco Cessation Employer Survey

This was a telephone survey of approximately 1,775 human resource or personnel managers in selected organizations in Arkansas in order to gather information regarding health resources that organizations make available to their employees for the improvement of their health.

Direct Service Workforce Survey

This was a survey of approximately 6,400 Employers and Attendant Care Providers/Personal Assistants that work in participant-directed programs or organizations that serve older adults, people with physical disabilities, and people with intellectual or developmental disabilities through Medicaid-funded programs. This survey will provide information about the home and community-based direct service workforce in Arkansas.

This was a web survey of all current UALR employees who regularly have contact with the Office of Financial Services for tasks in Accounts Payable, General Accounting, Payroll, and Purchasing. The purpose of this survey was to gather assessment information to be used by the UALR Office of Financial Services in evaluation of their services.
