A survey of approximately 12,400 Arkansas residents in each of the selected Arkansas counties. This survey is being conducted in order to understand a variety of general health practices, including information on health risk behaviors, preventive health practices, and health care access primarily related to cardiovascular disease and diabetes.

Arkansas Tobacco Quitline Evaluation Survey

DSSN Stroke Survey

This survey was sent to approximately 559 administrative personnel in hospitals across five states (Arkansas, Alabama, Louisiana, Mississippi, Tennessee) for the Delta States Stroke Network (DSSN). The purpose of this survey was to gather information from hospitals about their best practices in terms of stroke prevention and treatment protocols in order to better guide future stroke interventions and policies.

 

Ourtown Survey

This was a survey of approximately 53 high school students that participated in the Arkansas Ourtown Delegate Program. This was for the purposes of gathering attitudes and opinions from delegates in order to evaluate and improve the Ourtown program.

 

Women’s Health Waiver Survey

Infant Hearing Provider Survey
