IT Services forms should now be delivered to the Assistance Center

In the past, documents like account requests, OP-SCAN packets, and CEPRs were delivered to the IT Services front desk in SU-B 205. From now on, the IT Services Assistance Center is the primary contact point for the processing of all IT Services documents, including:

Unless otherwise noted, all forms may be filled out using a PDF reader, printed, signed, and then scanned back in for delivery via email.
