This application packet consists of the following:

  1. Student Information
  2. Release Form
  3. Travel Form
  4. Departmental Approval Signature Sheet
  5. Release and Waiver of Liability and Assumption of Risk
  6. Programs Abroad Participant Contract
  7. Medical Self Assessment
  8. Housing Information Form
  9. Supplemental Application
  10. Academic Recommendation
  11. Foreign Language Recommendation
  • In addition to submitting a completed application packet, you’ll need to do the following
  1. Submit a $100.00 application fee by the application deadline (make checks out to: UALR Programs Abroad/OIS)
  2. Submit 3 passport photos along with your application
  3. Attach your completed Supplemental Application materials
  4. Purchase an International Student Identity Card (ISIC) - This card may be purchased online at www.myisic.com for $25.00, or in the Office of International Services for $30.00. If your ISIC is purchased at our office, you will need to provide an additional passport photo, and bring a photo ID to verify your identity.
  5. Apply for your passport well in advance–it can take up to 3 months until it is processed and mailed back to you. Passport application and information is available at: http://travel.state.gov/passport/passport_1738.html
  6. Once you have your passport, bring it to the Office of International Services to have it photocopied for your file.

Do not return the passport application to the UALR Programs Abroad Office.
