2024 Application

I understand that, as a participant in Summer Laureate University for Youth (SLUFY), I have the responsibility to work to the best of my ability in my classes, that I will conduct myself appropriately and follow all rules and policies of the SLUFY program, that I will respect the property of others, and that I will respect the rights and privileges of all SLUFY students, faculty, staff, and others of the campus community. I understand that failure to comply may lead to program dismissal.

I have read the information on this application, including the above student statement, and hereby voluntarily and without coercion give legal representatives of SLUFY the irrevocable rights to use my child's name (or any fictional name), picture, portrait, likeness, photograph, or testimonials in all forms and media and in all manners, including composite or distorted representations, for advertising, trade, or any other lawful purposes, and I waive any right to inspect or approve the finished product, including written copy, that may be created in connection therewith. I hereby grant permission for the teacher and school to provide the materials required for submission of this application. I hereby authorize first aid care to be administered to my child as needed. In case of extreme emergency, I authorize ambulance or emergency services transportation to doctor or hospital.

I understand this application will not be processed until the $25 application fee is received by the Summer Laureate Office. I can pay 24/7 by visiting the SLUFY secure storefront link located at the SLUFY main page (ualr.edu/slufy) with credit or debit card, OR if I cannot pay this way, I will contact Lori Delk at ladelk@ualr.edu.
