The UA Little Rock Department of Human Resources is temporarily working remotely due to inclement weather.<\/strong> Due to these extenuating circumstances, we have created this portal for new employees to upload their identification documents during this time to satisfy the requirements for the I-9 Employment Eligibility Verification Form. <\/p>\n\n\n\n

As a new hire with the University of Arkansas at Little Rock, you are required to provide identification documents for the I-9 Employment eligibility Verification Form. A link to the list of Acceptable Documents for the I-9 Form is provided below. You will need a document from list A or a document from list B and C. <\/strong><\/p>\n\n\n\n

Please note all documents must be current (not expired) and signed to be considered valid. – Review the list of acceptable documents for the I-9 Form at: https:\/\/www.uscis.gov\/i-9-central\/acceptable-documents\/list-documents\/form-i-9-acceptable-documents<\/a> – The social security card is no longer required for the Personnel Data Sheet if it is not being used for the I-9 Form. However, the new hire must have a social security number to begin work. New hires may still want to present their social security card for payroll reporting and tax purposes but it is not needed as a separate requirement.<\/p>\n\n\n\n\n\n

Employees who are able to work from home have been encouraged to continue working; however, some employees who have been impacted by the COVID-19 pandemic may not be able to work for a number of reasons.<\/p>\n\n\n\n<p>The Families First Coronavirus Response Act (FFCRA) became effective April 1, 2020. Under the FFCRA all UA Little Rock employees who meet eligibility requirements may apply for up to 80 hours of paid sick leave. Additionally, UA Little Rock employees who have been<br>\nemployed with the university for 30 days or more may be eligible for expanded Family Medical Leave under the Act.<\/p>\n\n\n\n<p>In accordance with the Families First Coronavirus Response Act, UA Little Rock provides eligible employees up to 12 workweeks or 480 hours of paid job-protected leave, continuation of group health benefits, and reinstatement to the same or equivalent job upon return from leave.<\/p>\n\n\n\n<h1 class=\"wp-block-heading\" id=\"h-requesting-ffcra-leave\"><strong>Requesting FFCRA Leave <\/strong><\/h1>\n\n\n\n<p><a href=\"https:\/\/forms.gle\/mTaWTsrCwsRHWidz5\">FFCRA Leave Request Form<\/a>  <b>Be sure you are logged into your UA Little Rock Google account before accessing this form.<\/b><\/p>\n\n\n\n<h1 class=\"wp-block-heading\" id=\"h-required-documentation\"><strong>Required Documentation<\/strong><\/h1>\n\n\n\n<p>You must provide documentation in support of your paid sick leave to UA Little Rock as specified in applicable IRS forms, instructions, and information. UA Little Rock may also require you to provide additional documentation in support of your expanded family and medical leave taken to care for your child whose school or place of care is closed, or if your child care provider is unavailable, due to COVID-19-related reasons. <strong>All required documentation may be uploaded <a Leave<\/u><\/strong><\/p>\n\n\n\n<p>Below are the reasons you may apply for emergency paid sick leave and examples of appropriate documentation that should be provided with your request.<\/p>\n\n\n\n<p>\u274f 1) I am subject to a federal, state, or local quarantine or isolation order related to COVID\u201319.<\/p>\n\n\n\n<p><strong><em>Examples of documentation might include a city, state, or other governmental quarantine order.<\/em><\/strong><\/p>\n\n\n\n<p>\u274f 2) I have been advised by a health care provider to self-quarantine due to concerns related to COVID\u201319.<\/p>\n\n\n\n<p><strong><em>Examples of documentation might include a note from a health care provider or other documentation that you are under a doctor’s order to self-quarantine.<\/em><\/strong><\/p>\n\n\n\n<p>\u274f 3) I am experiencing symptoms of COVID\u201319 and seeking a medical diagnosis.<\/p>\n\n\n\n<p><strong><em>Examples of documentation might include a doctor’s note, or a statement from the employee that the employee is sick and has an appointment with a health care provider. <\/em><\/strong><\/p>\n\n\n\n<p>\u274f 4) I am caring for an individual who is subject to either number 1 or 2 above.<\/p>\n\n\n\n<p><strong><em>Examples of documentation might include a note from the sick person’s doctor or medical care provider.<\/em><\/strong><\/p>\n\n\n\n<p>\u274f 5) I am caring for my child whose primary or secondary school or place of care has been closed, or my childcare provider is unavailable due to COVID\u201319 precautions.<\/p>\n\n\n\n<p><strong><em>Examples of documentation might include a notice of closure or unavailability from your child\u2019s school, place of care, or child care provider, including <\/em><\/strong><strong><em>a notice that may have been posted on a government, school, or day care website, published in a newspaper, or emailed to you from an employee or official of the school, place of care, or child care provider.<\/em><\/strong><\/p>\n\n\n\n<p>\u274f 6) I am experiencing another substantially similar condition specified by the secretary of health and human services.<\/p>\n\n\n\n<p><em><strong>Examples of documentation include notes, emails, or notices related to your request for paid leave.<\/strong><\/em><\/p>\n\n\n\n<p><strong>All required condition, the employee must provide a statement from the healthcare provider stating that the employee is able to return to work.  The medical release must be submitted to the Department of Human Resources on or before the return to work date.  Any work restrictions must be identified on the statement and approved prior to returning to work.<\/p>\n\n\n\n<h1 class=\"wp-block-heading\"
