If you have any questions about the complaint process or need assistance completing this form, please contact the Title IX Coordinator at 501-916-5716, Monday through Friday from 8:00 a.m. to 5:00 p.m. or email [email protected]. You may make a report anonymously; however, this may limit the University’s ability to investigate the incident or provide resources to the involved parties.

If you would like to report confidentially, students may contact UA Little Rock Counseling Services at 501-916-3185 or UA Health Services at 501-916-3188. UA Little Rock employees may contact the Employee Assistance Program at 501-686-2588 or 800-542-6021. Additional local and national resources are available on the Office of Title IX  website. Roles
Complainant – any person alleged to have been harmed by the conduct reported on this form
Respondent – the individual who is alleged to have engaged in the conduct reported on this form
Reporting Party – the individual submitting the report. This may include individuals fulfilling their mandatory reporting obligations.
Witnesses – any individual who has knowledge of or information about the harassment or discrimination being reported.
