Incident Report

The UALR Incident Report form is an opportunity for any individual in the UALR community (students, faculty or staff) to provide information concerning an alleged Student Code of Conduct Violation. You may also email us directly using this address deanofstudents@ualr.edu or by completing the online form below.

Privacy: Information collected via this reporting system will be treated as private and is protected under the Federal Education Right to Privacy Act (FERPA) of 1974. The report is an educational record. The SCRS staff, an accused student, and those with an educational need to know will view the report.
