SUBJECT:       New Hire Documentation

WELCOME TO UALR!

As a condition of employment, payroll direct deposit is required for all new employees.  The direct deposit process has been refined and works with great reliability.  Prior to being set-up in the payroll system to receive your first direct deposit, you and your hiring Department must complete the appropriate paperwork.  Forms you are required to complete are listed below and are included in this packet:

 

1.            Personnel Data Sheet – Requires an original Social Security Card or Social Security Number Verification Form obtained from Social Security Administration (original Social Security card must be submitted to Human Resources within 90 days of employment);

2.            Federal W-4 Tax Form;

3.            State of Arkansas Tax Form Employee’s Withholding Exemption Certificate (AR4EC) OR State of Arkansas Tax Form Employee’s Special Withholding Exemption Certificate (AR4ECSP);

4.            U.S. Department of Justice Employment Eligibility Verification Form (I-9).  (List of acceptable documents are on the back of the I-9 Form);

5.            Acknowledgement Form regarding UALR’s Drug Free Workplace Policy;

6.            Direct Deposit Form (requires voided check or document showing savings account number with transit routing number);

7.            Acknowledgement Form regarding UA Board Policy 425.2.  (UALR employees are eligible to participate in employee only contributions to the Optional Retirement Program through a Supplemental Retirement Annuity (SRA) Plan);

8.            State of Arkansas Statement of Selective Service Status Form;

9.            Acknowledgement of Grant funded Position Form; and

10.        Acknowledgement of Benefit Policies Form.

11.        Form W-11 – Hiring Incentive to Restore Employment (HIRE) Act – required for all new hires who have not been employee for more than 40 hours during the last 60 days (request form in Human Resources).

Departmental Documentation for New Employee Processing

The documentation your Department is required to complete and submit for you depends on your employment classification as listed below:

1.                  Classified (full or part-time)

o   Electronic Personnel Action Form

o   Affirmative Action Review Form (copy of approved form)

o   Disposition Form

o   Personnel Requisition Form (copy of approved form)

2.                  Non-Classified/Administrative (full or part-time)

o   Electronic Personnel Action Form

o   Affirmative Action Review Form (copy of approved form)

o   Signed Job Offer Letter

o   Resume

3.                  Academic (9, 10, 10.5 and 12 month)

o   Electronic Personnel Action Form

o   Affirmative Action Review Form (copy of approved form)

o   Signed Job Offer Letter

o   Resume/Curriculum Vitae

4.                  Academic (part-time Faculty)

o   Electronic Personnel Action Form

o   Resume/Curriculum Vitae

5.                  Hourly or Work Study

o   Electronic Personnel Action Form

o   Position Classification Questionnaire (only if pay offered is more than $10.49 an hour)

6.                  Stipend Payments

o   Electronic Personnel Action Form

o   Stipend Classification Worksheet (if position is new)

If your position is Grant funded, your Department will submit the above documentation to The Office of Research and Sponsored Programs (ORSP) before forwarding to the Department of Human Resources.

Please understand that failure by you or your Department to complete or submit all required forms will prevent Human Resources from being able to process you in the payroll system and consequently, will keep you from receiving your first direct deposit.

SUBJECT:         BANNER Web for Employees

Great News!   BANNER Web for employee is now available for all employees to use.  This application allows employees to view and update records from the BANNER Human Resource database using WEB technology.  After you receive your first paycheck, you may access this BANNER feature of BOSS (Banner On-line Services System) from UALR’s web site at ualr.edu and click on register-on-line (BOSS), or from the Department of Human Resources’ web site at http://ualr.edu/humanresources and click on employee web (BOSS).

To login to BANNER Web for employees, input your UALR T-number and a six (6) digit PIN.  Your PIN will be the last six (6) digits of your Social Security Number.  However, if you are or have been a student, you may already have a PIN assigned by the Office of Admissions and Financial Aid (569-3110).  The system may require you to change your PIN after initial access.
