All IRB forms are in Microsoft Word. We only accept digitized requests. Scanned copies will not be accepted.
Protocol submissions should be:
- Filled out completely
- Signed electronically
- Dated correctly
- Submitted as a single PDF file
Include CITI course completion reports in your protocol submission. CITI completion certificates will not be accepted. To learn more about CITI training, please visit Required Training.
