Name Change

Students who need to change a name on their UA Little Rock academic records must submit a completed Name Change Request Form with two forms of identification.

For U.S. Residents, two forms of identification are accepted:

  • One Primary: Signed social security card, marriage license, or court order
  • One Secondary: Driver’s license, state-issued photo I.D., or Military I.D. card which has the new legal name.

For non-U.S. residents, acceptable forms of identification include an international passport, visa, or resident alien paperwork or identification card.

Gender Designation Change

To change a gender in the UA Little Rock system, complete and submit a gender designation change form by mail, email, or drop it off at our office in the Donaghey Student Services Center, Room 218.

The documentation required to make this change includes a scanned copy of a driver’s license or passport reflecting the new gender identity, a letter of support from a health professional, or a court order for the gender change.
