If personal information on your Forms I-20 or DS-2019 has changed or if they are found to be incorrect, please report the requested update with 10 days of the change.
Submit the SEVIS Update Form and select “I-20 / DS-2019 Correction or Change”. You will be required to attach documentation verifying that the update is warranted. You will receive notice by email when your updated I-20 or DS-2019 is available for you to pick up from the ISS front desk.
Name Change
The name on your Form I-20 or DS-2019 must be consistent on all documents. The I-20 or DS-2019 should list your name as listed on your passport. If your name changes due to marriage or other circumstance, first update your passport, then request an I-20 or DS-2019 update.
If you believe your name is incorrect on your I-20 or DS-2019, please demonstrate evidence of your proper name by submitting your passport. SEVIS requires a Last Name or Surname, with the option of a Given Name or First Name and an optional Middle Name.
