Immigration regulations require sponsors to monitor the health, safety, and welfare of exchange visitors. Supervisors or hosting faculty should complete this report every six months throughout the program, and one week prior to program completion.

Supervisors must maintain, at a minimum, monthly personal contacts with program participants. Such contact may be in-person, by telephone, or via exchanges of electronic mail (including a response from the participant) and must be properly documented. Sponsors must promptly and appropriately address issues affecting the participants' health, safety, and welfare identified through such contacts.
