The forms included on this website are used for a variety of offices on the UA Little Rock campus. Please read the information provided with each form.
These admission forms are used while students are applying for admission to UA Little Rock. For questions regarding any of these forms, contact the Office of Admissions at UA Little Rock. Call the Office of Admissions at 501-916-3127.
Authorization for Electronic Advising
This form gives an adviser the ability to communicate with the student via email communication. Students choosing to communicate with their adviser via email will need to complete this form and then submit it directly to the appropriate academic adviser in the School of Business.
D Grade Transfer Request Form
UA Little Rock will allow only 6 hours of transferred D grades to be submitted to a student’s degree plan. Students will need to complete this form and then have it approved by their adviser. Once the adviser approves the form, they will submit it to the Office of Records and Registration for processing.
Declaration or Change of Major Form
Once students are ready to declare a business major, they will need to complete the Declaration or Change of Major form. Once it has been completed, they should submit it to their School of Business adviser for processing.
Drop or Withdrawal Form
The drop or withdrawal form is used after the drop/add week ends at the beginning of each semester. If a student wants to drop an individual course or withdraw for the semester, they will need to complete this form and submit it directly to the Office of Records and Registration. The Family Educational Rights and Privacy Act (FERPA) allows students to declare which persons might be able to accompany them to their academic advising session.
- Via your BOSS account, under the Student Services tab, you may make changes to this through the Privacy Settings feature.
School of Business students can use this form to plan a term-by-term path to graduation based on the student’s expectations of number of classes to take per term (when work and family obligations are considered), whether or not Summer classes are taken, and so on. The student and advisor can review the plan to be sure courses are in sequence and to address any special scheduling concerns (like Fall- or Spring-only courses, etc.).
Technical and/or Military Credit Request Form
Students with previous Technical Credit or Military Credit will need to submit the following form so it might be evaluated and potentially added to their academic record at UA Little Rock.
Transcript Request Form
For students looking to submit an official or unofficial transcript to another agency or office, they will need to complete this Transcript Request Form. This is done through the Office of Records & Registration. For questions, contact the Office of Records and Registration at 501-916-3110.
