Health Plan Information Reporting/1095-C

Beginning with the 2015 tax year, the federal Affordable Care Act (ACA) requires all large employers to report to their employees and to the IRS, information on health plan eligibility and health plan enrollment.

If you were employed by the University at anytime from January 1, 2015 through December 31, 2015, AND you were eligible to enroll in the University’s health plan, you will receive a 1095-C form.  The reporting is required if you were eligible to enroll even if you never actually elected to enroll in health coverage.

Form 1095-C reports the Employer-Provided Health Insurance Offer and coverage.  If you were enrolled in the University’ health plan, this form documents the months that you (and any enrolled family members) met the federal requirement to have health coverage.

Next Steps:

  • Watch for the 1095-C form in the mail at your home address. It will be mailed by SOVOS Compliance, the firm working with the University to prepare the 1095C forms and IRS reporting.
  • The forms will be mailed to your home address no later than February 1, 2016 and should arrive in your mail by the middle of February, 2016.
  • Carefully review the form when you receive it. If you think the information on the form is incorrect, if you receive the form but need a replacement copy or if you do not receive the form, contact the Benefits in the Department of Human Resources at 501-916-3180 or email hr-benefits@ualr.edu for a corrected or replacement copy.
  • Use the form in completing your tax return or provide it to your tax account/preparer for their use. The Information on the form should match the health plan coverage information you report on your tax return.
  • The 1095C form will be provided only to employees. The University and SOVOS cannot distribute your form to any other individuals. You will need to share a copy of the form with your dependents if they will also file a separate tax return.
