Welcome to another fabulous year of Summer Laureate! We are celebrating the 45th anniversary of this wonderful program, so let’s make this year the best yet!
Below you will find the links to the various forms that we need for you to complete. Please fill out and submit these forms as soon as possible, but definitely by April 30:
The following form is to request copies. You may use this online form and upload the originals, or you may print out the hard copy Copying Request Form and bring it in person to the June 8 meeting with your originals. You will need a separate form for each set of originals.
All Copying Requests must be submitted by June 8, either physically at the meeting, or digitally through the online form.
If you do NOT need any copies, please fill out the hard copy form and check that box. We will have some copies of the form at the June 8 meeting.
